A Geriatric Care Manager is a health service professional who specializes in assisting families with their senior loved ones. They can act as an advocate, performing an assessment & evaluation of the Senior's current living situation, and even develop an individualized plan of care, and can provide recommendations to assist them to achieve the greatest quality of life.
These services are available by our pre-qualified third party providers. They are precisely for Seniors requiring help within their homes. The services range from : house cleaning, lawn service, snow removal, security systems to someone watching the family pet. We have an array of provided services. This is a service which helps assist families to identify the right type of senior housing including independent assisted living with services, alzheimers (memory supportive care), adult foster care, or skilled/ long term care nursing homes. Our highly trained and highly educated Placement Specialists can assist families in making the right choice for their loved one.
Many families do not recognize the services which can support seniors in their own home or a senior community. These can be skilled or basic care services, along with the deciphering through of the different insurances covered by Seniors.
